Payment is due at time of service.

If you are unable to make your appointment, please give the practice ample notice. Cancellation or no call no show for an appointment will charged $15.00. Cancellation or no call no show for a surgical appointment will be charged $50.00.

A flat fee of $5.00 is added to any prescription which requires packaging and shipping.
